What your medics get in the field.

Uncharted is an offline-first mobile app for pre-hospital and remote care. A medic documents a patient from point of injury through handoff, including MIST intake, serial vitals, medications and labs, then exports structured reports to the next level of care. No network connection is required at any step.

  • Fully offline
  • AES-256 encryption
  • HIPAA-ready
  • TCCC 2024
Uncharted vitals screen: serial vital signs table for a casualty with out-of-range systolic BP, heart rate, SpO2 and shock index flagged in red

From point of injury to handoff.

Pre-hospital care means hours, and sometimes days, between injury and surgical care. Uncharted follows that timeline, so the record keeps up without taking attention away from the patient.

  1. MIST intake in seconds

A new casualty opens straight into a MIST form. Speak the MIST instead of typing and it is transcribed on the device, with no signal needed, so the first entry takes seconds, not minutes.

    • Mechanism and treatment quick-add chips
    • Evacuation or triage category set at intake
New casualty MIST intake form with chief complaint, mechanism quick-add chips and casualty name
  2. Serial vitals with trends

Each set of vitals lands in a time-ordered table. Values outside the ranges you configure are flagged red the moment they are saved, and the trend chart shows where the patient is heading.

    • Out-of-range values flagged red as they are saved
    • Automatic MAP and shock index
    • Trend chart per vital with reference bands
Vitals trend chart plotting systolic BP, diastolic BP and MAP over four hours of care, with normal-range thresholds
  3. Drugs, interventions and reminders

Log every drug and procedure with dose, route, time and who gave it. Put a re-dose timer on any drug straight from its row, so the next dose is never left to memory.

    • One-tap chips for common drugs and procedures, from TXA to needle decompression
    • TCCC dosing hints, like TXA within 3 hrs of injury
    • Alarms that fire on the lock screen
Meds & TQ tab: TXA, fentanyl and ertapenem doses with times, and a tourniquet card showing 4 hours 15 minutes elapsed flagged in red
  4. Structured reports for handoff

The record becomes the report. Generate a PCR, SOAP note, MIST or 9-Line from what you have documented, or build a format of your own from a template, then sign it and hand it off.

    • PCR, SOAP, MIST and 9-Line
    • Build your own format from a template
    • A guided telemedicine call script for remote consults
Share tab: telemedicine call script, then PCR, MIST Report, 9-Line and SOAP Note under Reports, and Hand Off to Another Device
  5. Your protocols, on device

Keep the SOPs you actually use inside the app and read them with no signal. Add the people you call most, like your medical director, receiving hospital or dispatch, so they are one tap away.

    • Paste text or import PDFs
    • Add contacts from your phone or by hand
Protocols screen with a text SOP expanded and readable on the device
