Prolonged Field Care · iOS + Android
No pen. No paper. No problem.
Uncharted is an offline-first mobile app for pre-hospital and prolonged field care. Document a patient from point of injury through handoff — MIST intake, serial vitals, tourniquet timers, meds, labs — and export structured patient reports to the next echelon of care. No signal required at any step.

Serial vitals · critical flags · shock index — captured in-app
01 · The encounter
Point of injury to handoff.
Prolonged field care means hours — sometimes days — between injury and a surgeon. Uncharted is built around that timeline, so the record keeps pace with the patient instead of competing with them for your attention.
MIST in under a minute
New patient opens straight into Mechanism, Injuries, Signs/Symptoms, Treatment — plus OPQRST and SAMPLE history. Structured fields, no typing marathons.
Hours of care, on the record
Serial vitals with trend charts, auto shock index, and critical-value flags. Tourniquet timers computed from wall clock — they survive restarts. Drugs, fluids, labs, and vent settings logged as you go.
Structured reports, ready for handoff
Export the complete record — preformatted MIST, SOAP note, 9-line, Patient Care Report (PCR), or a full call audit log. Ready for secure transfer.

03 · Onboarding
Set your standard of care, once.
A guided walkthrough gets a new medic running in minutes — and it lets agency medical directors define the usage profile, clinical thresholds, and key contacts that shape how the whole team documents care. Configure it once; every medic deploys the same.
One profile, the whole app adapts
Choose Military/Tactical or Civilian/Remote and the app reshapes itself — terminology, injury mechanisms, and triage categories all match how your service actually works.
Your protocols, your thresholds
Medical directors set the triage and evacuation thresholds that flag critical values, so every record reflects your agency’s standard of care — not a generic default.
Key contacts and MEDEVAC, preloaded
Load the contacts and evacuation details your medics reach for, so they’re in place before the first call — not scrambled for mid-incident.
The same setup, every medic
A guided walkthrough gets each medic running in minutes on the same configuration — identity, profile, and thresholds consistent across the team, and replayable anytime from settings.
04 · In the field
Built for efficiency in the most challenging environments.

05 · Security
HIPAA-ready. Offline by design, not by accident.
Patient data never leaves the device — there is no server to breach. Security is enforced in the architecture, from the encrypted store to the lock screen.
Encrypted at rest
Every patient record, the audit log, and your settings live in a SQLCipher AES-256 encrypted database. Photos and audio stay in the app’s private sandbox under OS file protection.
Hardware-backed keys
The database key is generated on device and held in the iOS Keychain or Android Keystore.
Locked at the router
Biometric unlock with PIN fallback. Patient screens cannot render while the app is locked — cancel means locked.
Tamper-evident audit log
Every create, open, edit, export, and delete is hash-chained. Any alteration of history breaks the chain.
No cloud. Period.
No sync, no telemetry on patient data, no third-party analytics touching PHI. The record exists where you are.
Photos stay in the sandbox
Wound photos never touch the camera roll. EXIF metadata — including location — is stripped before save.
